Thank you for choosing to be a part of the Living Way Church family! We ask that you supply the following information so that we can better minister to you. Church directories, whether digital or print, are for internal distribution only and are never shared outside Living Way Church membership. Birth dates and anniversary dates are not published in digital or print directories. The head of household should complete this form and use the Add Adult and Add Child buttons to supply information about household members who will be attending Living Way Church.
